All such investigations are conducted in a fair and expeditious manner in accordance with Department Policy, along with State and Federal Laws. You may fill out and print the Citizen Complaint Form below. Then sign the form, and mail or hand deliver the form to:

All Citizen Complaint Forms must be signed to be valid. False reporting in an attempt to unjustly subject an employee to undeserved discipline or slander, or place his/her employment in jeopardy, can result in criminal charges.
